Job Overview
We are seeking an experienced Adult Social Worker to join our Integrated Hospital Discharge Team, contributing to a high-performing, collaborative service dedicated to effective patient transitions.
Please note: This role requires a Social Work qualification and a minimum of two years of post-qualified experience.
About the Team
You’ll be part of a dynamic, fast-paced team operating within a hospital setting. The team plays a key role in supporting adults through the discharge process, carrying out Mental Capacity Assessments, Safeguarding enquiries, and Care Act 2014 responsibilities. The service embraces a hybrid work model, offering flexibility between remote work and onsite collaboration to ensure both efficiency and work-life balance.
About You
You must hold a Social Work Degree, DipSW, or CQSW, with at least one year of post-ASYE experience. You will be expected to conduct comprehensive assessments under the Care Act 2014, applying a strengths-based approach to develop effective, person-centred solutions. Previous experience in hospital discharge planning, community work, and complex case management will be highly advantageous.
